Please print out this form and mail it to the address below.
- I
agree to follow all rules and regulations during the entire tour-trip.
- I will remain with the group and the bus that I am assigned to during the entire trip, and during the tours at each college.
- I am solely responsible, including financially responsible, for any damage to hotel room or damage to any other facility I attend. I am also responsible for any room service or telephone calls that are made from my hotel room.
- I will meet all curfew times to be in my room, and get up in time to be on the bus at the designated time. I will be reasonably quiet while at the hotel. (Zero tolerance here).
- If I need assistance, I will inform an adult sponsor.
- I will remain in the room that I am assigned to unless I am approved by the bus leader.
- I will refrain from foul language (cursing), illegal behavior, and consumption of alcohol, illegal drugs, cigars, cigarettes, or chewing tobacco.
If caught doing any of these listed above, you may be sent home from the nearest bus depot at the expense of your parents.
